Out of whack is a great way to say that something is not normal!
It’s a very American construction and also very informal.

  • I am so tired at the moment, my sleep pattern is all out of whack. 
  • This weather is out of whack! It’s raining and it should be sunny!

So in your example, if you’re tired in the afternoons it may be that your blood sugar is not at a normal level.
