The sentence is correct but you must put a question mark at the end of the sentence.
Do you wish to watch TV or to go to bed early?
Also, instead of saying “Do you wish to…” I would say “Would you like to…” or “Do you want to…” as it sounds more natural and it is more often used by native speakers.
